Forms The forms below are required for participation in the Professional Internship Program. The Professional Internship Program has new policies for the
2014--2015 academic year. Please review the eligibility and stipend
sections carefully. Students Who Are First-Time Program Applicants Students who are first-time Program applicants must complete the following application forms and have their internship approved by the director of the Program before they can begin "clocking" hours toward their stipend. It is the responsibility of the would-be intern to make sure the application is complete. Be ready with the information required to complete the Program's Application Form and keep a copy of all forms. An application form is required for most opportunities, including jobs, graduate school, grants, fellowships, and/or residencies. Two Faculty Letters of Recommendations are required. Choose your recommenders wisely. This is an opportunity to cultivate relationships with faculty for future opportunities, such as graduate school, residencies, and teaching. Learn more about letters of recommendation by reading the guidelines we have organized for faculty. We consider administrators to be part of the "general faculty"; if there is an administrator who can speak to your readiness for an internship, such as someone you have worked with at Cooper, his or her recommendation is suitable (again, think of who might be a credible reference for a future opportunity). However, at least one recommendation must be from one of your professor. Read the Letter of Understanding with Students Participating in Professional Internship Program carefully and sign it. We hold interns to the details! Present the internship opportunity and learning experience to cooper's program director and staff to receive formal approval via an in-person meeting. We need the Internship Site Information & Checklist Form to confirm the internship, document internship placements, and send evaluations. Interns use timesheets to track their own internship hours. Submit the signed form to the Career Center, 6th Floor. Please submit internship timesheets to the Career Center based on the Payroll Schedule. Interns can only be paid according to the payroll schedule. However, interns do not lose the time worked if they turn the timesheet in after a specific pay period. Please note that there is a final deadline marked on the payroll schedule. Student graduating in the fall semester have a December deadline. If an intern turns a timesheet in after the final date, he or she forfeits the stipend. Submit the W-4 Tax Form to the Business Office, 7th Floor, 30 Cooper Square; students only need to do this once during their career at Cooper. click here to collapse « |
